Endovascular Management of Acute Total Occlusion of the Aorta

Abstract

|
Acute total occlusion of the aorta is a rare and clinically critical disease, that aggressive immediate diagnosis and treatment are mandatory. 73 year-old male visited our hospital complaining of painful sensation on both legs. Under likely impression of acute total occlusion of the aorta, emergency abdominal aortogram revealed total occlusion of distal aorta, both iliac arteries. Intra-arterial thrombolysis by urokinase was attempted for 48 hours because intervention during acute stage is highly related to distal thromboembolic event. Follow-up angiography showed improved arterial flow, but residual stenosis at both common iliac arteries. Self expandable nitinol stents were inserted at both iliac arteries by v-kissing technique. Angiography showed satisfying outcome without any complications. The patient was discharged without any combined medical problems. The use of intra-arterial thrombolysis with percutaneous transluminal angioplasty is an alternative strategy of conventional surgical approach in treating acute total occlusion of the aorta
